Job DescriptionJob Description

Essential Duties & Responsibilities:

Community/Provider Liaison position is primarily responsible for providing community outreach for Healthy Start Coalition of Orange County. This includes working with OB/GYN, public and private hospitals and birthing facilities to ensure the successful implementation of the Healthy Start prenatal and postnatal screening.

This position reports directly to the Executive Director.

Specific Activities:

  • Offering technical assistance and training to prenatal and infant health care providers (including the health department, federally qualified health centers, fee-for-service, managed care and hospitals) on legally required Healthy Start screenings; ways to make prenatal and infant health services more consumer friendly; ways to improve communication on patient risks and needs across providers; and community maternal and infant health related issues and outcomes.
  • Developing and implementing a consumer marketing and education plan. At a minimum, this plan should include strategies for: increasing Healthy Start screenings and acceptance of Healthy Start services; educating consumers on choosing a health care provider, patient rights and responsibilities; assuring at least one community-wide consumer forum to identify continuing issues and barriers to care for the pregnant and infant population; assuring at least one consumer focus group to identify continuing barriers to accessing pre and postnatal services; and developing at least one patent education product to be distributed to consumers and/or providers.
  • Communicating with DOH- Orange, the Orange County Healthy Start Coalition and its Marketing and Community Standards Committees regarding issues related to consumer and provider knowledge of Healthy Start prenatal and postnatal screenings, best professional practices for the delivery or prenatal and infant health care and related psychosocial services, patient education and Medicaid funded maternity and infant health care services. The Community Liaison will collaborate with DOH – Orange and its committees to design and implement strategies to address issues identified by providers, consumers and the Coalition.
  • Attending conferences, in-services and other training to maintain professional competency and update knowledge; preparing monthly summaries for the Coalition and engagement with providers and consumers and other activities.
  • Provide courier service to Providers to ensure timely input into data system on a weekly, monthly basis
  • Create social media and other marketing practices to effectively communicate with providers, community, Coalition and other community partners.
  • Maintains comprehensive log of all provider communication, education, visits, and materials distribution
  • Maintains relationships with OB/GYNs, hospitals and birthing facilities and promotes Healthy Start programs through weekly, monthly, or quarterly visits as needed
  • Prepares and distributes quarterly Healthy Start screening report/screening feedback to providers and Healthy Start special reports and educational material as may be needed
  • Research data and update community reports
  • Updates provider contact information regularly

Other:

  • All other duties as assigned by the Executive Director.

Knowledge, Skills and Abilities

  • Knowledge of basic programmatic management
  • Skilled in the use of Excel, Word, Outlook, and other Microsoft office applications
  • Ability to interpret and apply federal and state guidelines to local situations
  • Ability to meet and correspond professionally with contracted providers

Education and/or Work Experience Requirements:

  • Bachelor’s degree (preferred, not required) in social services, public health, maternal and child health services or related field with at least three or more years’ experience preferred; or, 4 or more years’ experience in social services, public health, maternal and child health services, marketing or related field.
  • A master’s degree from an accredited college or university can substitute for one year of the required experience.

Additional Eligibility Qualifications:

  • Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary leadership.
  • Must be able to travel to attend conferences, training and other events, as required.
  • Ability to work flexible traditional and non-traditional hours.
  • Ability to establish and maintain effective working relationships with Coalition partner agency staff, Board committees, volunteers, staff, donors and the general public.
  • Knowledge of Healthy Start, and other local health and human service providers.
  • Ability to work independently and to carry out assignments to completion within parameters of instructions given prescribed routines, and standard accepted practices.
  • Must be able to communicate effectively and be a team player at the Coalition.
  • Must be able to commit to the Coalition vision and be passionate about its mission.
  • Requires reliable transportation as travel is required, valid driver’s license, good driving record and automobile insurance.

Physical Requirements:

  • Ability to safely and successfully perform the essential job functions consistent with the ADA, and other federal, state and local standards.
  • Must be able to travel to alternate locations.
  • Required to sit, stand, walk, speak and hear while performing the duties of this job, regularly. The position requires extensive computer use so the employee must have sufficient hand dexterity to use a computer keyboard and be capable of reading a computer screen
  • Lifting/carrying up to 25 pounds.

This position is dependent upon the continuation of funding. As such this position may terminate when the funding is no longer available.

Salary: $43,000.00 per year
