  • Lead and facilitate treatment team to develop and carry out individualized care plans for all patients, prior authorizations for insurance, complete psychosocial assessments, discharge planning, process group, communication with families and facilities.
  • Performs other duties as assigned.
Requirements
  • Ability to work every other weekend and holiday rotation.
Education/Certifications
  • Bachelor's degree in social work is required.
  • Current Social Worker Licensure from the State of Ohio is required.
Experience
  • Background in social work with 2-4 years of experience is preferred.
  • Experience in inpatient psychiatric setting preferred.
