As a Patient Care Coordinator, you are not just a coordinator – you are responsible for the patient care plan, supporting caregivers and families with empathy and patience. To be a successful Patient Care Coordinator, previous healthcare experience and a background in handling inbound and making outbound calls will be great Comfortable using software applications for data entry and client information look-up, such as, but not limited to EMR/EHR & CRM systems.
