Job DescriptionJob Description

Purpose of Position:

Under the oversight of a Registered Nurse or Provider, the Pediatric LPN Referral Coordinator will provide support and back up to the Pediatric, RN Case Manager. Assisting with prevention, outreach, care coordination, scheduling, order entry for necessary lab, radiology, travel or other duties as needed per the patient’s plan of care.

Essential Functions:

  • Assume a leadership role for pediatric prevention and wellness to include but not limited to Well Child Checks (WCC) and Immunizations
  • Work in collaboration with the Vaccine Coordinator to run reports for pediatric immunizations and Well Child due lists for Nome. Assist with Village WCC /Immunization lists as needed for village travel
  • Work in collaboration with the RN Pediatric Case Manager to provide support for well and chronically ill children in the region
  • Provide immunizations in the Nome Primary Care Clinic
  • Scheduling Pediatric appointments as needed
  • Travel with the pediatric team to provide immunizations in all clinics in the region as needed
  • Assist with travel and housing arrangements as needed and within scope
  • Process patient referrals from NSHC and other providers, assisting with travel and housing arrangements as needed
  • Work within their scope, the referral coordinator will assist the Pediatric RN Case Manager (CM) with lab order entry, radiology order entry, obtaining medical reports from other facilities /providers and coordinate / schedule patient appointments in Nome or the outside facility as appropriate. Assist with transfer of the patient’s NSHC records to the planned delivery facility / provider if outside of Nome.
  • Ensure patient reminders are up to date to reflect proper follow up with the Primary Care Provider (PCP) and Specialty Providers
  • Assist in planning and organizing meetings for Pediatric Immunization Team
  • Assist with travel orders for appropriate patients as needed
  • Assist with planning and organizing specialty clinics and other events in support of healthcare promotion
  • Support RN Case Manager with maintaining patient contact and other duties as needed
  • Assist RN case manager with collecting pediatric statistics for reporting
  • Assist in the Primary Care clinic as needed
  • Perform other duties as assigned
