Constellation Hospice is looking for volunteers to visit patients who have a terminal illness.  Make a difference in your community, be part of the hospice team, and have a flexible volunteer schedule. One hour a week can make a big difference to someone nearing the end of life


Tuck-In Volunteer Description: 

  • Provides telephone calls to patients and families residing at home utilizing a Tuck-in script and tracking log to ensure that they have supplies, medications, equipment, and pain control prior to the weekend. 
  • Provides companionship and emotional support for patients and family members during regular telephone calls.
  • Provide telephone calls to patients and families who reside at home prior to the weekend, checking on needs and providing support.
  • Utilize the Tuck-in call list from EMR.
  • Utilize Tuck-in script and tracking log to document patient needs.
  • Handoff documented patient needs to assign staff members, making sure that pain/symptom control issues are brought to the nurse's attention right away.

 

Requirements:

  • Must be 18 years old or older
  • Able to commit for one year to our program
  • Ability to listen deeply, interest in and compassion for others, good boundaries and self-care, meet people where they’re at in energy and tone
  • Online training takes ~ 20 hours and is personally enriching, focusing on personal death awareness and hospice philosophy
  • No close death in the last year
