Lemay Ave. Health & Rehab located at 4824 South Lemay Avenue, Fort Collins, CO, is a beautiful facility and provides services including 24-hour skilled nursing care, physical, occupational, respiratory, speech-language therapy, secure memory units, group and individual activities, and transportation to medical appointments.

We are accepting applications for a Social Services/ Discharge Planner able to work:

  • Full Time (40 hours per week)
  • Monday through Friday
  • 8:00 am to 5:00 pm

Compensation

  • Starting wage between $20.00/hour - $24.00/hour (depending on experience)

Social Services/Discharge Planner

  • Serves as patient/resident advocate
  • Collaborates closely with the other facility departments including nursing and providers to the ultimate benefit of the patients/residents
  • Collaborates closely with community resources
  • Coordinates and attends patient/resident care conferences
  • Attends and participates in interdisciplinary meetings
  • Upholds confidentiality of all information provided by patients, residents, families, or facility personnel
  • Works with clinical staff to ensure appropriateness of placement, level of care, and unit
  • Documents patient/resident information with consistency and accuracy to meet specific goals and objectives
  • Cooperates with expectations related to being a mandated reporter of abuse and/or neglect
  • Offers support and counseling related to social and emotional factors and crisis intervention
  • Creates individualized care plans for each resident
  • Facilitates residents and family access to facility and community services
  • Coordinates discharge and any needed community services upon discharge from the facility
  • Assists residents and their families with resources pertaining to discharge planning

Social Services/Discharge Planner Requirements

  • Bachelor's degree from an accredited college or university with a major in a human behavioral sciences field. Examples of degree titles include Social Work, Gerontology, Human Development/Family Studies, Human Services, Psychology, or Sociology required
  • Case management experience preferred
  • Prefer one year's experience in a Geriatric setting but not necessary
  • Clinical knowledge of medications, cognitive levels, and an understanding of mental health diagnosis/behaviors
