Who We're Seeking: Are you a dedicated and compassionate Medical Social Worker (MSW) looking for a fulfilling opportunity to make a real difference in the lives of hospice patients? We are currently seeking a dynamic individual to join our interdisciplinary Hospice team, where you'll have the unique opportunity to provide direct, one-on-one support to individuals in the comfort of their homes.
Your Responsibilities: As a member of our dedicated team, you'll:
- Directly engage with patients in the comfort of their own home, providing one-on-one support.
- Work collaboratively with a supportive interdisciplinary team, including medical doctors and caregivers, to develop personalized care plans.
- Maintain consistent, timely, and accurate communication with all members of the patient’s health care team.
- Regularly evaluate each individual's progress toward their care goals, updating the care plan as needed in collaboration with the healthcare team.
- Participate in team meetings and collaborate with supervisors to identify challenges, and foster supportive and innovative solutions.
- Deliver evidence-based interventions tailored to address psychosocial challenges impacting clients’ medical care.

Requirements:
- Master’s or doctoral degree from a school of social work accredited by the Council on Social Work Education
