Job Title: Intake Coordinator

Job Summary:

Employees in this role are considered support staff for office management needs. This includes secretarial duties and clerical responsibilities. Employees are also responsible for the tracking and monitoring of all YCCS referrals and performing other Manage Care Entities process and procedures. This includes assuring performance specifications are followed, marketing of program, analyzing data, and reporting on referrals, waitlist, and discharges.

Duties and Responsibilities:

  • Provide courteous and timely customer service.
  • Adhere to Agency and program policies and procedures.
  • Initiate and complete all administrative paperwork included in the Client Intake Packet.
  • Ensure that intake paperwork meets quality assurance standards [as determined by Division Director or designee] and is prepared for entry into the client chart.
  • Respond to all intake calls and voicemails within two business days.
  • Orient client to service availabilities, rights, and responsibilities.
  • Ensure client ID is assigned, documented, and entered into any and all databases and relevant systems.
  • Provide guidance and incidental [“dotted-line”] supervisor to any assigned support staff assisting with intake duties.
  • Communicate in a timely manner with the Practice Manager when additional resources or support is needed.
  • Assist with phone calls and translations as needed.

Minimum Qualifications:

  • High school degree or equivalent; post-secondary education an asset
  • At least 2 years of clerical experience
  • Bilingual skills in Spanish-English required
  • Microsoft Office proficient (e.g. Excel, Word, Power Point)
  • Flexibility to work shifts, including evenings, as required
  • Ability to organize, prioritize and work under extreme work pressure, heavy workloads and deadlines
  • Positive attitude and work ethic
  • Excellent verbal and communication skills
  • Must be team oriented
