Nurse Ambulatory Case Manager

The RN Ambulatory Case Manager will report directly to the Director of Care Coordination Services. They will work collaboratively with physicians, nurses, and other members of the healthcare team to ensure optimal care coordination for pediatric patients in the ambulatory setting.
The Ambulatory Care Coordination team provides comprehensive care coordination services to pediatric patients and their families across various specialty clinics and outpatient departments. The team is committed to delivering high-quality, patient-centered care that promotes positive health outcomes and enhances the patient experience.
Job Responsibilities:
- Assess and evaluate the needs of pediatric patients and their families to develop individualized care plans
- Coordinate and facilitate appointments, referrals, and follow-up care for patients within the ambulatory setting
- Collaborate with healthcare providers to ensure seamless transitions of care and continuity of services
- Provide education and support to patients and families to promote self-management and adherence to treatment plans
- Utilize evidence-based practice guidelines and standards to optimize patient outcomes and resource utilization
- Participate in interdisciplinary care team meetings and case conferences to discuss patient progress and care coordination strategies
