NP Orthopaedic Shoulder

Under the supervision and direction of the Physician Co-Chief of the Shoulder Service, Advanced Practice Provider (APP) Director, and Administrative Manager, the Nurse Practitioner (NP) is responsible for the care of the Orthopaedic Shoulder patient population in the outpatient setting. The NP coordinates all care to enhance the entire patient surgical experience, which includes diagnosis and treatment of non-operative and operative conditions, preop optimization, postop care, and ongoing surveillance.The NP will conduct independent clinics to enhance new patient access, and for all other visit types (urgent, preop, established, and postop). The NP will practice in accordance with the rules, regulations, and standards of nursing practice issued by the state and federal governing boards, including the Academy of Nurse Practitioners (NPs) and the American Nurses Credentialing Center (ANCC) or the American Association of NPs (AANP).
Principal Duties And Responsibilities
- Provides direct care, counseling, and teaching for the Shoulder patient population.
- Functions independently in own clinic and manages/coordinates all aspects of patient care (x2 per week, 15-20 patients per clinic).
- Assists the Co-Chief of Shoulder and other service physicians with overall management of their patients and the organization/workflow of their clinics.Works with surgeon in clinic x1-2/week)
- Assists APP colleagues with some coverage during vacations, sick or FMLA absences.
- Performs complete histories and physical examinations on new, established, preop, and postop patients.
- Handles urgent add-on or walk-in patient visits.
- Orders, interprets, and evaluates appropriate laboratory and diagnostic tests.
- Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings.
- Consults with supervising physician/other Shoulder surgeons regarding abnormal findings outside NP scope of practice.
- Prescribes medications and narcotics as necessary according to organizational and state/federal regulatory policies and procedures.
- Initiates consultations and referrals for patients.
- Obtains preoperative surgical consents; confirms laterality and abides by all hospital consent guidelines.
- Performs surgical optimization/education for patient and family, reinforcing the surgical plan presented by the attending physician.
- Identifies social determinants which may affect postop recovery/rehab and plans accordingly, e.g., contact case management prior to surgical date.
- Communicates with PCP, all medical specialists including our geriatric POSH clinic, and anesthesiologists to optimize patients prior to surgery.
- Orders necessary preoperative diagnostic tests (MRSA/MSSA for appropriate surgeries), imaging, consultations, and procedures as indicated or requested by the attending physician to prevent delay with surgical procedure, which also includes obtaining medical clearance and instruction regarding medications/anticoagulation prior to surgery.
- Provides symptom management for Shoulder patients; surgical and nonoperative.
- Performs shoulder injections and/or orders it to be done with our Musculoskeletal radiologists.
- Manage high volume of patient calls, Patient Gateway messages, emails and completes all outpatient clinical forms (FMLA, insurance disability, hardship needs).
- Obtains prior authorizations for surgery, scans, medications, or DME when peer-to-peer calls are necessary (insurance MD-NP calls).
- Consistently provides high quality and timely documentation of clinic visits (within 72 hrs), admission note from office, outpatient progress notes, procedure notes, and e-mail/phone conversations.
- Contributes to patient presentation in clinic, weekly indications conference, and multi-disciplinary meeting/conferences as necessary.
- Identifies patients appropriate for division research studies and obtains consents after completion of Insight Training. May conduct independent research and/or quality improvement studies.
- Practices cost effective medicine in an efficient manner, maximizing available resources.
- Discusses patient data with other physicians and professionals as appropriate in a multidisciplinary setting.
- Maintains communication with all inpatient staff, as necessary (APPs, case management, OT/PT, consults if necessary), Waltham OSC staff for day surgery patients, and all involved in the transition of patient care (skilled facility, VNA, PCP, referring physicians).
- Provides an environment for effective patient/family centric care.
- Demonstrates expert clinical judgment skills to function in an independent role.
- Performs other responsibilities required by the Orthopaedic department/Shoulder division.
- Demonstrates professional collaborative and consultative relationships with all Shoulder service staff and other multidisciplinary providers.
- Participates in departmental and organizational committees and programs as appropriate and attends all required departmental meetings, in-services, and professional trainings.
- Maintains superior interpersonal and communication skills as a member of the healthcare team to collaborate effectively with patients, families, all Shoulder service staff, and community health workers.
- Remains actively involved in continuing education with a commitment to self and departmental growth.
- Adheres to all established safety policies, clinical policy and procedures, and infection control precautions; identifies potential or actual unsafe situations in the environment and takes measures to rectify the situation.Documents and complies with hospital standards of care.
- Must have the ability to work in a high stress, dynamic, outpatient clinical setting with changing organizational climate.
- Must have the ability to be self-directed and demonstrate initiative in addressing the responsibilities of the position.
- Must have the ability to utilize appropriate problem-solving and conflict resolution skills.
- Must excel in effective and positive communications with patients, families, nurses, case managers, surgeons, trainees, support staff, and many interdisciplinary providers.
- Must be energetic and quality driven and have a proven ability to be a productive member of a team dedicated to providing optimal patient care.
- Must be proficient with MGH computer systems as needed to independently perform daily tasks (Epic preferred).
- Holds self and colleagues accountable to learn, develop and achieve the best and highest quality results.
- Treats colleagues with respect.
- Must maintain the highest degree of professional judgment under the collaborating supervising physician, director of APPs and practice manager.
Work may require contact with needles and other sharp instruments or division specific technology. Prolonged standing and fast-paced environment can be expected.
Qualifications
- The appropriate candidate will be a graduate of an accredited NP school with ANCC or AANP Board Certification.
- Current Commonwealth of Massachusetts RN license with authorization to practice in the expanded role (NP).
- Relative to first assist in the operating room, the candidate must have graduated from an RNFA accredited course and completed a patient OR caseload to obtain first assist hospital privileges.
- Must have or be eligible for and maintain a current DEA registration, Massachusetts Controlled Substance Registration (MCSR).
- Qualified in all basic and advanced cardiac life support standards as established by the American Heart Association.
