A Day in the Life:
As a Case Manager in the Emergency Department, you'll begin your day coordinating patient care transitions and ensuring seamless transfers to appropriate clinical services or discharge destinations. You'll engage in discharge planning for patients with complex needs, collaborating with an interdisciplinary team to arrange follow-up care. You'll also provide consultation to patients, families, and hospital staff on discharge planning processes and regulatory requirements, while leading support staff in case management tasks. Your role includes participating in departmental program planning, goal setting, and process improvements, as well as maintaining detailed documentation and handling third-party reimbursement processes.

About the Job:
  • Pay: $76.83 base + $115.25 OT; blended $84.51
  • Schedule: 10-hour evening & 10-hour night shifts
  • Shift Time: 11:00am - 9:30pm; 4:00pm - 2:30am; rotating w/ weekends
  • Start Date: June 9, 2024
  • Department: Emergency Department
  • Duration: 3 - 6 month contract
  • Location: Palo Alto, California

Qualifications:
  • Experience as a Case Manager in an Emergency Department and/or acute Level 1 - Level 3 hospital
  • Required Certification:
  • Bachelor of Science in Nursing (BSN)
  • BLS - Basic Life Support (American Heart Association)
  • CA Registered Nurse license
