Requirements:

• Board Certified - REQUIRED if Board Eligible (ABIM ONLY)
• Active WA license or IMLC (LOQ in hand/must upload during name clears) - REQUIRED
• Completed Endocrinology Fellowship - REQUIRED
• Malpractice - Clean Preferred
• Availability at time of name clear - REQUIRED
• ACLS/BLS/DEA - REQUIRED
