Under the general supervision of the VP of Direct Clinical Services, the Case Manager - Outpatient Clinics is responsible for managing an assigned caseload of members having behavioral health needs by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches; and monitoring staff performance.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

  1. Manages an assigned caseload of members having behavioral health needs.
  2. Assesses member needs, evaluates treatment options, creates treatment plans, coordinates care and monitors member progress.
  3. Develops, monitors, and evaluates treatment plans and progress.
  4. Provides core care management services by providing comprehensive care management, care coordination and referrals to community and social support.
  5. Plans and coordinates care management needs regarding medical, psychiatric, substance use, crisis intervention, legal, housing, education/employment, entitlements, social, and peer support services.
  6. Conducts screenings and assessments of high-risk needs such as substance use, psychiatric symptoms, harm to self/others, abuse/neglect, or domestic violence.
  7. Conducts monitoring and follow-up services.
  8. Facilitates rehabilitative and treatment services.
  9. Provides comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs.
  10. Manages discharge plans upon completion of treatment.
  11. Works collaboratively with members, families, physicians, and nurses to ensure high quality care.
  12. Maintains the member's comprehensive clinical record through detailed documentation.
  13. Arranges for the intervention of social services, health, or governmental agencies when needed.
  14. Determines the need for therapeutic, psycho-social, or psychiatric evaluations based on member progress.
  15. Interacts with members to ensure they are informed about their treatment plan and answers any member's questions and concerns.
  16. Writes case reports.
  17. Works with agencies and service providers to facilitate case coordination and information sharing.
  18. Coordinates the integration of the social services/case management function.
  19. Reviews treatment goals and resources.
  20. Participates in the development and implementation of member care policies and protocols.
  21. Promotes effective and efficient utilization of clinical resources.
  22. Participates in regular interdisciplinary staff meetings.
  23. Provides face-to-face reviews with members and/or relevant staff on a regular basis regarding member progress so that treatment plans can be modified, as necessary, to ensure that the treatment goals and objectives are being achieved.
  24. Participates in interagency planning and service coordination activities directed to improve and enhance service continuity and effectiveness for members.
  25. Monitors staff performance.
  26. Performs related duties as assigned.

  1. Knowledge of DWIHN policies, procedures, and practices.
  2. Knowledge of the DWIHN provider network and community resources.
  3. Knowledge of the Michigan Mental Health Code.
  4. Knowledge of MDHHS policies, rules, regulations, and procedures.
  5. Knowledge of Federal policies, rules, regulations, and procedures as it relates to DWIHN.
  6. Knowledge of MHWIN.
  7. Knowledge of the LOCUS model.
  8. Knowledge of the clinical care process (screening, assessment, treatment planning, case management, and continuing care).
  9. Knowledge of and ability to use screening and assessment tools for behavioral health services.
  10. Knowledge of the continuum of care for all SED, SMI, Co-Occurring Disorders.
  11. Knowledge of Outpatient, Co-Occurring services, CM, ACT, Supported Employment, and Med Drop.
  12. Knowledge of behavioral health case management practices and principles.
  13. Knowledge of Integrated Care services for behavioral health patients.
  14. Knowledge of Medical Necessity Criteria for Behavioral Health Services.
  15. Knowledge of the Federal Confidentiality Regulations, 42 CFR, Part 2.
  16. Knowledge of Motivational Interviewing.
  17. Knowledge of documents / regulations that govern the provision of mental health services, e.g., Medicaid Manual Mental Health and Substance Abuse Chapter III, State Plan for Medicaid, Michigan Department of Health and Human Services Quality Plan, BBA requirements and the Mental Health Code.
  18. Knowledge of the practices and principles of psychological, emotional, and sociological assessment and diagnosis.
  19. Knowledge of crisis training and experience in de-escalation, i.e., suicide prevention, development of crisis plans, trauma informed care, etc.
  20. Knowledge of the identification and treatment of co-occurring mental health and substance use disorders.
  21. Knowledge of community mental health services and resources.
  22. Knowledge of the DSM-IV Statistical Manual.

REQUIRED EDUCATION:

A Bachelor's Degree from a recognized college or university in Social Work, Nursing, or a related field.

REQUIRED EXPERIENCE:

Two (2) years of professional clinical experience in a behavioral healthcare or mental health setting including at least one (1) year of experience performing case management in a behavioral healthcare or mental health setting.

REQUIRED LICENSE(S):

A Valid State of Michigan clinical licensure: LBSW, RN.

A valid State of Michigan Driver's License with a safe and acceptable driving record.

WORKING CONDITIONS:

Work is usually performed in an office setting but requires the employee to drive to different sites throughout Wayne County and the State of Michigan.
