Position Title: Primary Care Provider - FNP/PA

Your Role with the Company :

* M-F, 32-40 hrs/week
* This FNP/PA will be seeing all ages with a focus on the quality of patient care in a mobile clinic
* Working in a collaborative team setting to provide primary care as well as health coaching and education to help individuals manage their chronic conditions for a better quality of life
* FNP/PA will make a big impact in the community that they serve providing continuity of care to their patients
