Sign On Bonus: $9,000.00 (Conditions Apply)

The posted compensation range of $35.70 - $49.78/Hour is a reasonable estimate that extends from the lowest to the highest pay Pathways Home Health & Hospice in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Pathways Home Health & Hospice may ultimately pay more or less than the posted range as permitted by law.

POSITION SUMMARY:
The Home Health Licensed Vocational Nurse (LVN) is responsible for providing skilled nursing care to assigned patients and their families. Documents observations and interventions related to the patient’s physical, mental, social, and emotional status. Communicates patient status and needs based on a plan of care with the Clinical Team Manager and other members of the home care team. Performs duties consistent with the Vocational Nurse Practice Act.

AREAS OF RESPONSIBILITY:

  1. Provides those services requiring the skills of a licensed vocational nurse in accordance with the plan of care.
  2. Documents observations, clinical findings, problems, skilled interventions, goals, and discharge plans.
  3. Provides and documents skilled nursing care on all visits (includes skilled observation of the patient’s condition, skilled care, procedures, and teaching of the patient and/or family).
  4. Communicates with the Clinical Team Manager or with the physician and other members of the home care team, informing them of significant changes in the patients’ condition and needs. Documents these communications.
  5. Attends and actively participates in the clinical team multidisciplinary patient conference. When attendance is not possible, provides appropriate input on assigned patients.
  6. Submits all documentation according to agency requirements.
  7. Meets established productivity standards.
  8. Uses agency-provided telecommunication devices (i.e., cell phone, voice mail, and pager) according to established guidelines.
  9. Follows established standards for point of service technology, documentation, and synchronization.
  10. Seeks out and maintains skills and knowledge relevant to skilled home care nursing.
  11. Participates in agency-sponsored in-service education.
  12. Participates in agency-sponsored quality improvement activities.
  13. Assists in the development of agency protocols, procedures, and policies as requested.
  14. Assesses, organizes, and delivers teaching materials for assigned home care patients as requested.
  15. Performs other duties as assigned consistent with skills and training and the mission and goals of Pathways.

QUALIFICATIONS:

  1. Preferred: IV Therapy and Blood Withdrawal Certification
  2. Minimum of one year of nursing experience.
  3. Demonstrates willingness and ability to work with electronic input and telecommunication devices.
  4. Current California driver’s license and automobile insurance with minimum Bodily Injury Liability coverage per agency policy.
  5. Current CPR– American Heart Association “BLS Health Care Provider” requested upon renewal.
