Elara Caring is looking for a passionate *Licensed Practical Nurse LPN* to join our elite team of healthcare professionals and make a difference, one patient at a time.

*Responsibilities*
* Travel home to home within a coverage area and provide direct patient care, including catheterization, tube feeding, and basic life support as needed.
* Conduct patient assessments and maintain accurate electronic health records (EHR) to ensure compliance with HIPAA regulations.
* Collaborate with healthcare teams to develop and implement individualized care plans.
* Educate patients and families on health management and treatment options.
* Maintain effective communication with patients, families, and other healthcare professionals to ensure optimal care delivery.

*What is Required?*
* Current LPN license as required by state
* 1 year of experience in an acute care or equivalent setting
* CPR certification with American Heart Association or America Red Cross
* Positive attitude
* Dedication to quality patient care
* Strong communication skills
* Reliable transportation to perform job duties
