Rockville Centre, United States Better Home Health Care Agency, Inc. Full time

The Home Health Aide (HHA) is an employee trained to provide personal care and related services in the home. A HHA Carries out health care tasks under the supervision of a Registered Nurse (RN) and may also provide assistance with personal hygiene, housekeeping and other related supportive tasks to a patient with health care needs. The Home Health Aide reports to the RN in charge of the case for patient care and to the Staffing Coordinator/Team Leader for administrative aspects.

 

RESPONSIBILITIES:

A.            Routine
1.      Follow instructions on task-oriented Plan of Care.

2.      Assists patient in bathing, toileting and dressing.

3.      Assists patient with care of mouth, hair and ordinary care of nails.

4.      Assists patient in shaving.

5.      Assists patient in walking and transfer activities.

6.      Shops, prepares normal diet and assists patient in eating.

7.      Takes temperature, pulse, respiration and blood pressure as directed.

8.      Assists patient to physician(s) and medical facilities as instructed.

9.      Documents care and reports observations.

10.   Performs light housekeeping tasks such as:

a. Changing bed linen

b. Dusting and vacuuming rooms used by the patient.

c.  Dishwashing and tidying kitchen.

d. Tidying bedroom and bathroom.

e. Doing patients personal laundry (may include mending and ironing).

f.   List needed supplies.

g. Assists with payment of bills and other errands.

B.      Other Health Related Tasks
1.      Assists with skin care as prescribed for patient

2.      Prepares meals according to special dietary needs/ complex diets.

3.      Assists with exercises as instructed.

4.      Reinforces dressings and does simple non-sterile dressing changes on stable skin surfaces.

5.      Performs and records urine test for sugar and acetone, and tests for bacteria.

6.      Measures intake and output as directed.

7.      Assists with “ostomy” care on mature ostomies.

8.      Assists with the application and/or use of devices such as prosthetics, braces, crutches and canes.

9.      Assists patient with medications.

10.  Helps patient’s re-learn household skills.

C.    Special Circumstances
Tasks permissible under special circumstances may only be performed for a patient whose characteristics and case situation meet all of D.O.H. criteria.  A field RN will train and document proficiency in employee folder.  Each HHA must receive training in the exact skill and/or procedure to be performed with each patent.  Training and competency evaluation in the performance of these tasks are not transferable from patient to patient, and are at the discretion of the agency:

1.      Administration of medication.

2.      Administration of pre-filled insulin, subcutaneous

3.      Assist with gastronomy feedings.

4.      Assists with postural drainage.

5.      Assists with TENS.

6.      Irrigate indwelling catheters.

7.      Change drainage bag/Collect urine specimen.

8.      Administer commercially prepared enemas or douches.

9.      Assists with urinary diversion device.

10.  Heat and Cold Applications

11.  Sitz Baths.

12.  Administration of prescribed medications to nebulizer.

13.  Set/Regulate the flow of oxygen.

14.  Take an apical pulse.

15.  Assists in diabetic blood testing.
