*Accountabilities and Responsibilities:*

To assist the resident in those activities related to personal care.

1.) To assist to bathe or assist with partial bath, shower or tub bath. Note: There will be scheduled shower/bath days for each resident along with a record to obtain optimum hygiene requirements.

2.) To assist with grooming: Oral hygiene, hair care, hand and feet cleaning and basic skin care.

3.) To assist with clothing, dressing and undressing.

To assist with normal elimination: kidney and bowel function.

1.) To assist to and from the bathroom.

2.) To use appropriate procedures for commode, bedpan, urinal and incontinent products.

3.) As ordered by the residents’ Primary Care Physican and in accordance with the individuals care plan, procedures for optimal bowl elimination by maintaining stool chart and following protocol per standing orders. In-service procedures will be provided for these procedures.

To assist the resident with transfers and mobility issues.

1.) To assist with ambulating with a walker, cane, and use of other assist devices.

2.) To assist with mobility, transfers from bed to chair and/or wheelchair use.

3.) To use proper body mechanics at all times for the residents and avoiding personal body injury to ones self.

To maintain a clean and safe environment, and to know the importance of both, in caring for all the residents.

1.) To practice universal precautions.

2.) To perform appropriate infection control techniques for all residents and yourself.

3.) To perform specified house cleaning each day by using a daily check off list provided by your supervisor. Cleaning shall include by not limited to: Cleaning all bathrooms, bedroom floors, vaccum and/or sweeping floors and rugs, bed changing and making up, laundry, dusting rooms, may need to wipe up a messy floor, etc.

4.) To identify and report any hazards or unsafe conditions in the facility and report such events to your supervisor and/or administrator.

*5.) *To know and follow the policy on handling emergencies. For all medical emergencies: calling the ambulance, administrator, physician and/or the RN consultant. Appropriate telephone numbers are posted for all emergencies.

*Diatary Requirements:*

1.) To assist the dietary cook with menu planning, preparation and serving.

2.) To assist individual residents’ with eating.

3.) Observe and report and record changes in resident’s usual eating and drinking habits.

4.) Help cleanup after meals and keep the dining area clean at all times.

To provide optimal resident satisfaction while meeting all facility policies and state-governed regulations as outlined in this job description.

1.) To effectively communicate with residents’ and families, including those with physical, emotional or mental impairment.

2.) To perform all services which support the resident’s rights.

3.) To behave in a positive professional and cooperative manner with administration, residents’, and residents family members.

4.) To maintain the confidntiality of all resident and family information.

5.) To perform all tasks for which he/she has been instructed.

6.) To report any resident grievances to your administrator.

7.) To report to work at least 15 minutes prior to your shift start time so you can review the MAR’s and resident notes prior to your shift starting. Also, you will obtain a shift report from the person on shift.

8.) To report to work on time and if you cannot do so, notify the facility in a timely manner so an alternate staff person can be called into fill the vacant shift.

Observe and report, any unusual or significant changes in a resident’s condition or status.

1.) To observe and report changes (i.e., vomiting, pain, shortness of breath).

2.) To observe and report changes in behaviors (i.e), mood swings, crying, withdrawal, hyperactivity, assaultiveness).

3.) To observe and report changes in noted mental states (i.e.), disorientation to person, place, and time, impaired judgement).

4.) To notify your supervisor and/or administrator of observed changes, and document in the daily notes before the end of scheduled work hours.

5.) Report to your supervisor and/or administrator any resident’s accidents or incidents immediately. (i.e., resident falls, call for the ambulance). In the event of an emergency transfer, the resident should have a packet ready, which contains the following information: resident’s face sheet, transfer sheet, and advanced directive. Always copy the resident’s MAR as this goes with the resident. Please give all information to the ambulance EMT.

A copy of the employment policies shall be given to the employee upon hire. An employee handbook will also be given outlining specific policies, benefits and confidentiality information.

*Minimum Qualifications*

1.) 18 years of age or older

2.) Physical strength sufficient to perform a variety of bending, some moving, and lifting (no more than 30 pounds) and other minor supportive activities.

3.) Good interpersonal skills.

4.) Demonstrated commitment to quality job performance (2 to 3 References Required).

5.) Able to multi-tak a plus.

6.) Must have a background free from criminal convictions for crimes against persons, including without limitation, theft, abuse, neglect or sexual misconduct.

*Medication Administration*

To administer medications you must have attended an approved DHHS medication class and poses a CRMA Certificate from the State of Maine. The facility posts all staff member’s certificates to be seen. This is a good practice allowing for residents, family members of residents and outside visitors so they can become familiar with all staff members on-site. An 8-hour medication refresher course is required every 2 years. All referesher class moneies are paid for by the facility. We currently have our RN consultant, who is a state-certified medication trainer; do all our in-services and refresher training or enroll you in one of the State of Maine Certified Trainers.

*Reporting Sequence: *

Any questions and/or issues need to be directed to your facility administrator. An agreement will be reached in a timely manner and reported back to you. All issues may not be resolved to everyone’s liking. We strive to handle each circumstance on an individual basis and look at all alternatives such as meeting the best interests of the facility and other staff members. Your patience and understanding is greatly appreciated.

Job Type: Full-time

Pay: $15.00 - $17.00 per hour
