Role Description

This is a full-time on-site role for an MDS Coordinator. The MDS Coordinator will be responsible for conducting assessments, creating care plans, and coordinating with the interdisciplinary team to ensure accurate and timely completion of the Minimum Data Set (MDS) assessments. They will also be responsible for monitoring and updating care plans, as well as ensuring compliance with regulatory requirements.


Qualifications

  • Licensed Practical Nurse (LPN) (RN) certification
  • Experience in care planning and creating care plans
  • Knowledge of the MDS assessment process
  • Understanding of geriatric nursing principles
  • Excellent organizational and documentation skills
  • Strong attention to detail
  • Ability to work effectively as part of an interdisciplinary team
  • Experience in long-term care or rehabilitation settings is preferred
