Care Case Manager

Summary of the Role:

The Care Case Manager is primarily responsible for providing services to clients who are HIV positive. The Care Case Manager will coordinate services for Ryan White eligible clients in both Health District 1 and Health District 2 for the State of Idaho. The position is full-time, requiring 40-hours per week, with some travel. The Case Care Manager will work closely with clients living with HIV/AIDS who may have psychosocial and/or health related needs. Case Care Management services is a strength-based approach to service that includes treatment adherence, counseling, coordination and follow-up of medical treatments, client advocacy, and assistance in obtaining housing, financial support, legal services, social support, and other needed services. The goals of Case Care Management are to ensure that clients with HIV/AIDS have timely access to comprehensive medical care and social services, prevent disease transmission, minimize HIV progression, and to promote and support client independence and self-sufficiency. 


Responsibilities:

  • A typical workday/week is Monday-Friday 9:00am-5:00pm, yet occasionally during events, weekend hours maybe required.
  • Handle sensitive information in a confidential manner.
  • Adhere to HIPPA regulations.
  • Maintain a caseload and provide case management daily.
  • Complete comprehensive documentation of intakes, needs assessments, care case plans, yearly recertifications, and referrals to service providers from eligible clients.
  • Keep comprehensive, confidential files (a must).
  • Sustain a current knowledge of HIV/AIDS medical treatment and disease trends.
  • Retain proper documentation for client’s goals and methods for reaching goals.
  • Participate in conference calls and training in and out of the state as requested.
  • Work with Supervisor Case Manager on administrative reporting.
  • Prepare and submit monthly billing statements.
  • Successfully complete all training as requested.
  • Perform HIV/HEP-C testing and counseling.
  • Maintain current Driver’s License and current vehicle insurance.
  • Accompany clients to medical appointments as needed.
  • Perform other duties as assigned. 


Qualifications:

  • A bachelor’s degree in a health or human services field is preferred; or extensive experience defined as specialized case management training.
  • For social work degree must provide current Idaho State license.
  • Must pass state background check.
  • Must possess a valid Driver’s License and current insurance.
  • Must have own transportation.
  • Professional appearance and demeanor.
  • Must be culturally competent.
  • Excellent customer service skills.
  • Excellent written and verbal communication skills.
  • Excellent time-management skills and the ability to prioritize work.
  • Must be a self-starter and can work effectively under limited daily supervision.
  • Team player.
  • Strong organizational and planning skills.
  • Must be deadline oriented.
  • Robust command of Microsoft Office, social media, and internet searching.
  • Computer/Typing skills.
  • Discretion and good judgement.
  • Flexible attitude and the ability to deal well with changing assignments and priorities.
  • Ability to lift 30 lbs.
