Key Responsibilities:

Data Architecture Design and Implementation:


Develop and implement scalable data architecture solutions tailored to population health management.

Design data models and databases to store and manage large volumes of healthcare data efficiently.

Create and maintain documentation for data architecture, data flows, and data models.

Population Health Data Management:


Integrate diverse data sources, including EHRs, claims data, and social determinants of health data.

Ensure the quality, integrity, and security of population health data.

Develop and manage ETL processes for data extraction, transformation, and loading.

Analytics and Reporting:


Collaborate with data analysts and scientists to develop analytical models that support population health strategies.

Design and implement data visualization and reporting solutions to present actionable insights to stakeholders.

Monitor and evaluate the effectiveness of population health initiatives using data-driven metrics.

Data Integration and Interoperability:


Ensure seamless data integration and interoperability across various healthcare systems and platforms.

Implement data integration solutions that comply with industry standards and regulations.

Work with IT teams to ensure data systems are scalable and can accommodate future growth.

Collaboration and Communication:

Qualifications:


Bachelor’s or Master’s degree in Computer Science, Data Science, Healthcare Informatics, or a related field.

Proven experience in data architecture, with a focus on healthcare data and population health.

Strong knowledge of healthcare data standards, regulations, and interoperability frameworks.

Proficiency in data modeling, database design, and ETL processes.

Experience with data visualization tools like Tableau, Power BI, or similar.

Proficiency in programming languages such as SQL, Python, or R.

Preferred Qualifications:
