JOB DESCRIPTION

Insight Global is looking for Community Health Workers for a healthcare client of ours. As a Community Health Worker, you will work directly with members to help them navigate health care ecosystems to promote mental, physical, and social wellness. You’ll be at the front end of an innovative program which incorporates community health workers and virtual care in the context of a primary care practice. You will assess, define, and implement care plans individually tailored for each member. In partnership with the care team, you will work on increasing the patient’s knowledge of health and health risks, improving current health practices, promoting services, and supporting preventive activities with community collaboration. Candidate will be responsible for servicing members within a 1-hour driving radius of their home. This is a fulltime opportunity with shift times as follows: Monday-Friday, 8am-5pm PST.


Responsibilities:

- Engage the community, actively leading membership enrollment, and responding to inquiries from prospective members to guide them through the enrollment process.

- Actively engage with a panel of assigned members, to help them increase their health literacy.

- Collaborate with a multidisciplinary team to care for a panel of patients, ensuring they are up to date on preventive measures and their care plans.

- Provide care navigation, assisting members with: appointment logistics, prescription drug support, lab support, referral coordination, care plan adherence, sharing information with the care team, and providing resources.

- Co-develop care plans that address both clinical and social needs, understanding social determinants of health that are affecting the patient.

- Screen individuals for Social Determinants of Health (SDoH) and refer them to community resources for services such as financial assistance, legal aid, housing, food, or education.

- Coach patients using an action-planning model based on motivational interviewing techniques.

- Document services provided in client records and required databases.

- Communicate with patients via phone, video conference, and text messaging, involving other members of the multidisciplinary care team as needed.

- Provide health education on acute and chronic disease management through various services, including presentations, workshops, trainings, and small group discussions.


REQUIRED SKILLS AND EXPERIENCE

- 1+ years of working experience in health promotion or related job (community health, project coordination, social research, or administration in a healthcare setting).

- Basic conversational Spanish-speaking skills (don't need to read/write it)

- Comfortable working with multiple computer applications simultaneously

- Personal vehicle for transportation

- Valid CA driver's license


Exact compensation may vary based on several factors, including skills, experience, and education.

Benefit packages for this role will start on the 31st day of employment and include medical, dental, and vision insurance, as well as HSA, FSA, and DCFSA account options, and 401k retirement account access with employer matching. Employees in this role are also entitled to paid sick leave and/or other paid time off as provided by applicable law.
