Essential Functions (include, but are not limited to ):
1.Direct supervisor of assigned case management staff which includes but is not limited to recruiting, onboarding, goal setting, performance management, etc.
2.Perform comprehensive assessment of clients' health care needs within guidelines mandated by the Children 1st/ 1st Care program.
3.Implement and provide oversight for 1st Care Plans of Support
Minimum Qualifications : Bachelor’s degree in Nursing from an accredited college or university AND one year of nursing experience AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR Associate’s degree in nursing from an accredited college or university AND Two years of nursing experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification
Preferred Qualifications : Bilingual English/Spanish. Experience with case management, children and adolescents with chronic medical conditions and special healthcare needs.
Work Conditions & Physical Demands : Ability to carry objects with weight not to exceed 50 lbs., maneuver stairs without difficulty due to home visitations.
Salary Information : $65, Annually / Pay grade M
