Performs routine (journey-level) public health nursing work in the clinical or non-clinical setting. In collaboration with the Immunization Program, tracks, resolves, and coordinates care for vaccine preventable disease. Responsible for case management of perinatal hepatitis B cases in Public Health Region 2/3. Works directly with the Regional Perinatal Hepatitis B Coordinator, regional nurses, local health departments to provide delivery of services. Provides nursing consultation for individuals and their families. Provides direct public health nursing clinical services for immunizations, tuberculosis and adult health as needed in the region. Works under general supervision of the Assistant Regional Nurse Manager, Regional Nurse Manager and Regional Medical Director with moderate supervision and limited latitude for the use of initiative and independent judgement. Member of the Incident Command Structure in response to public health emergencies. Must be able to independently travel throughout the State of Texas and out-of-state to complete essential job functions. Must comply with HHS, DSHS, Texas Department of Public Safety applicable policies/procedures/standards. This position is office-based, Monday-Friday, 8:00 am – 5:00 pm. Essential Job Functions:
Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned.
CASE MANAGEMENT: Performs case management activities related to perinatal hepatitis B prevention in Public Health Region 2/3. Receives lab reports of HBsAg positive and unknown status results; determines pregnancy status and locates infected mother. Educates mother on hepatitis B transmission, case management process and elicits household and sexual contacts. Assures post partum serology testing is done to determine the mother’s infection status. Assures infants born to HBsAg positive and status unknown mothers receive the hepatitis B immune globulin (HBIG) and the first hepatitis B dose within 12 hours of birth. Performs case management of infants born to HBsAg positive and status unknown mothers including susceptible household and sexual contacts. Assures serology testing of infants and contacts to determine infection status. Documents all case management activities in the appropriate reporting and case management data base for submission to DSHS Central Office. Works closely with Regional Perinatal Hepatitis B Coordinator and physicians to identify HBsAg positive pregnant women as well as ensure infants receive the hepatitis B vaccine according to schedule. Must be able to independently travel throughout the State of Texas and out-of-state to complete essential job functions. (45%)
EDUCATION AND TRAINING: In collaboration with the Regional Perinatal Hepatitis B Coordinator, may provide assistance in assessing community needs; may provide approved educational activities using state approved training presentations and brochures for health professionals to include hospitals, birthing centers, laboratories, and prenatal care providers on hepatitis B transmission, reporting laws, vaccination schedule, case management, surveillance methods and contact investigations. Provides consultation, training and technical assistance to staff in PHR 2/3. (30%)
DIRECT SERVICE PROVISION: Provides public health nursing services in the clinic, home, and community. The nurse will utilize components of the nursing process in assessing, planning, implementing, and evaluating the physical, emotional, and psychosocial health status of clients, families, and communities. Provides individualized care according to policies, procedures, program standards, physician orders and standing delegation orders which may include lab testing, home visits, providing/administering medications, education and counseling, case management, referrals, and follow-up. Performs communicable disease intervention, surveillance, and prevention functions around Tuberculosis, HIV/STD, Immunizations, and other identified diseases. Documents service delivery using agency approved forms appropriate for the program. (20%)
REGIONAL SUPPORT AND DISASTER RESPONSE/RECOVERY: Participates in regional assignments/duties/special initiatives. Actively participates and/or serves in a supporting role to meet the agency’s obligations for continuity of operations activation during disaster response and recovery. Requires completion at minimum Incident Command System , , and courses. Required to work other than normal business hours, an alternate shift pattern and/or locations in order to respond to public health emergencies. Must be able to independently travel throughout the State of Texas and out-of-state to complete essential job functions. Maintains compliance with DSHS Policies and HHS Fleet Operations and Management Procedures and Standards, and a current valid Texas Driver's License. (5%)
Knowledge Skills Abilities:
Knowledge of nursing techniques and procedures; public health laws and regulations; community health and nursing care principles, practices, and procedures; and quality assurance principles and procedures. Knowledge of hepatitis B virus, perinatal transmission including screening guidelines, case management procedures and contact tracing.
Skill in using reasoning to identify problems, considering appropriate resolution of situations, collaboration patients, families and the community to address community needs and provide program specific and public health information, skill in the use of a computer and applicable software.
Ability to plan, coordinate, and evaluate the delivery of direct public health services; to provide patient care; to prepare and maintain records; to conduct community assessments; to communicate effectively via written and verbal communication: to provide independent home visits: to travel in the state of Texas.
