Summary

The Patient Aligned Care Team (PACT) Registered Nurse (RN) Care Manager will utilize the nursing process and evidence based practice to deliver and manage comprehensive care to an assigned panel of patients in a clinic setting. The RN is a key member of the PACT and multidisciplinary team and is responsible for collaboration with other team members, promoting effective communication and ethical practice. RN functions within their scope of practice to provide high quality direct patient care.

The Patient Aligned Care Team (PACT) Registered Nurse Care Manager demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. The nurse collaborates with the Core Team (Patient, Primary Care provider, PACT/RN Care Manager, PACT LPN and Medical Support Assistant) and Expanded Team (family/caregiver, internal and community based services in providing care to the patient) in developing the patient driven holistic care plan for life. The nurse collaborates with services internal and external to the VA to facilitate care transition to effectively meet the patient's needs. The PACT RN also functions as a part of the broader Primary Care Clinic team and has effective customer service skills when working with internal and external customers.

The primary duties for the RN - PACT position include but are not limited to:

  • Provide prescribed medical treatment and personal care services to ill and injured Veterans while following the PACT model of care. Provides efficient care triaging patients according to established protocols documenting observations, assessments, and changes in patient's condition.
  • Collaborating with health team members to facilitate positive patient care outcomes.
  • Providing a full range of nursing care to patients with a variety of physical and/or behavior problems.
  • Educating patients about healthy practices and treatment modalities.
  • Coordinating the care across settings and among caregivers and managing information.
  • Communicating effectively with internal and external customers, including, but not limited to: demonstrating professional speaking and writing abilities and documenting interactions in a responsible, accountable and ethical manner that will facilitate quality review and promote effective care management.
  • Complies with health, safety and environmental rules and procedures. Performs work in a manner that enhances the safety of the work environment. Ensures strict adherence to federal, state, and local codes concerning health, safety and the environment as applicable within the performance of his/her duties.
  • Assist each patient to identify and meet basic health needs and improve his/her health status to the individual's maximum potential.
  • Participates in unit/program level quality improvement processes and initiatives, as well as, customer service program.
