Summary

The VA Northeast Ohio Healthcare System is actively recruiting for a Social Worker to serve the Psychological Recovery Resource Center (PRRC), at the Cleveland (CRRC). This outpatient assignment is recovery based and focuses on the psychosocial and psychotherapeutic needs of clients diagnosed with severe mental illness. The position requires extensive knowledge in psychiatric diagnosis, discharge planning, relapse prevention, evidence-based therapies and VA and community resources.



Duties

The Social Worker duties include, but are not limited to:

  • Completes psycho-social assessments and military history. This information gathering will be directed toward identification of Veteran challenges and potential solutions.
  • Assesses veterans potential of harm to self or others
  • Assesses the veteran client's ability to provide for their daily life and psychosocial needs.
  • Responsible for participating in treatment plans that reflect a course therapeutic intervention that will address the needs identified in the assessment.
  • Acts as an advocate preserving the right of Veteran autonomy and choice.
  • Provides support and educates and empower every Veteran client to advocate for self.
  • Exercises a variety of individual and group counseling techniques that are evidence based in the treatment of mental health and demonstrate good clinical practice.
  • Acts as part of an interdisciplinary team, working collaboratively with other colleagues, providers and professionals.
  • Coordinates care between providers, professions, programs and services is managed efficiently and effectively, maintaining the Veterans right to privacy, confidentiality and the sharing of information.
Work Schedule: Monday through Friday, 8:00 a.m. to 4:30 p.m.
