Our collaborative and team oriented group of 55 providers serve at 11 convenient locations throughout Northeast Indiana.Locations include: Auburn, Columbia City, Fort Wayne, Huntington, Wabash, Kendallville, New Haven, with the addition of Warsaw in 2021.
