DUTIES
Home Health Care Service includes Home Based Primary Care (HBPC), Community Health and the Medical Foster Home program. HHCS operates out of the John L. McClellan Memorial Veterans Hospital with the main office in West Little Rock and a satellite office in Hot Springs. The incumbent will be responsible for providing primary medical management to a population of homebound, chronically-ill Veterans who reside within the HBPC service area. HBPC is an interdisciplinary model of care that provides a blend of primary and palliative care to a largely geriatric population of mostly rural dwelling Veterans. The incumbent provides comprehensive assessment, treatment planning, ongoing monitoring and care, patient/family education and discharge planning. The incumbent serves as primary care provider for specialized smaller PACT team as well as the collaborative physician for 1 APRN PACT team and others as needed for coverage. The incumbent works collaboratively with the other members of the team including registered nurses, rehab therapists, social workers, clinical pharmacists, clinical psychologists, dietitians, and a chaplain. The incumbent helps design and participates in service performance improvement initiatives and helps maintain a state of readiness for Joint Commission accreditation surveys. HBPC is an academic setting and the incumbent will coordinate education and training for geriatric fellows and allied health professions including graduate nurses, graduate social workers, pharmacy residents and students and psychology interns. The incumbent reviews and approves certain aspects of non-VA community care. The incumbent also provides backup for the HBPC Medical Director and other HBPC physicians.

Work Schedule: 7:30 AM to 4:00 PM Monday –Friday/No Call/No Weekends
50% travel within Central Arkansas Area (Conway, AR, Jacksonville, AR, and Cabot, AR)

Preferred Experience

Requires current and valid driver's license without 2 moving violationsBoard Certification in Internal Medicine or Family PracticeBoard Certification in Palliative CareBoard Certification in GeriatricsExperience with patient/family and professional health education;Experience with interdisciplinary health care teams;Home health experienceAdvanced computer skills.
