Clinical Assistant

Employees in this classification provide patient care management to an assigned clinic in the areas of clerical and clinical duties. In clinical setting, the employee receives direct supervision from physician or nursing supervisor in either oral or written form. In office setting, the employee receives direct supervision from department administrator. Work is performed under usual office and clinic conditions. May be stressful when working under emergency conditions. This position must supervised by a Registered Nurse.
Major/Essential Functions
- Promotes and reinforces patient centered medical home concepts with patients, their family and clinic personnel under the supervision of a nursing supervisor.
- Serves as the contact point, advocate and informational resource for patients, care team, family/caregiver(s) under the supervision of a nursing supervisor.
- Assists with the development, revision and coordination of the Plan of Care through collaboration with the multidisciplinary treatment team to meet the patient's needs effectively and efficiently; under the supervision of a nursing supervisor.
- Assesses, plans, implements, coordinates and evaluates the effectiveness of the patient.
- Develops short term and long term strategies in the development of expected patient outcomes; collects data through patient tracking in order to measure outcomes; under the supervision of a nursing supervisor.
- Promote, assess and evaluate the educational needs and requirements specific to your job function; under the supervision of a nursing supervisor.
- Ability to collaborate with an interprofessional team; under the supervision of a nursing supervisor.
- Knowledge of value based network quality care gap /HEDIS measures reporting using electronic clinical data systems Knowledge of the Healthcare Effectiveness Data and Information Set (HEDIS) Knowledge of importance of HEDIS in today's healthcare system; under the supervision of a nursing supervisor.
- Attends and participates in Care Coordination TEAM/and staff meeting.
- Participates in pre-visit planning and care planning for patients using value-based care quality metrics/
- guidelines as instructed; Assists with the development, revision and coordination of the Plan of Care through collaboration with the multidisciplinary treatment team to meet the patient's needs effectively and efficiently; under the supervision of a nursing supervisor.
Other duties as assigned by supervisor.
Required Qualifications
Education:
- A minimum of a High School diploma or equivalent.
- Graduation from an approved certified program for assistants in the area of required specialty.
- Certification by an approved accredited certifying agency in the specialty that particular work will be performed.
License must be Certified Nursing Assistant (CNA) or Certified Medical Assistant.(CMA). Must be licensed to practice in Texas.
