Duties

VA Portland Health Care System (VAPORHCS) is a Level 1A interdisciplinary teaching and quaternary facility with service-lines serving primary care, emergency medicine, medicine, surgery, specialty, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, geriatrics and multiple community-based outpatient clinics (CBOC). VAPORHCS operates a Community Resource and Referral Center which serves homeless Veterans in downtown Portland and also provides Home-Based Primary Care.

The incumbent practices as a RN within the Community Living Center (CLC), located at the Vancouver campus in the Rehabilitation and Long Term Care (RLTC) Division. The CLC provides short term transitional care for residents who require skilled nursing care, close medical monitoring and sub-acute rehabilitation. Resident populations include those having chronic or sub-acute medical conditions, including residents that are chronically ventilated. The RN is accountable for patient driven care of a defined group of residents. Based on education and experience, duties include, but not limited to:

  • Assignment as a charge nurse, preceptor; or in the interdisciplinary leadership roles of Veteran Care Manager, Minimum Data Set Coordinator, or Admission Coordinator.
