Duties
The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran. This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being.
Major duties include, but are not limited to:
- Demonstrates the ability to provide nursing care based on utilization of the health history and physical assessment skills and provides assistance for less experienced staff.
- Incumbent will be able to set up or reprocess Reusable Medical Equipment (RME) as applicable and be certified annually on the specific equipment for which they are responsible.
- Utilizes appropriate resources and tools to document, track, and assure follow through on patients with high risk needs or care management coordination.
- Plans with patient, family or caregiver, the specialist, the PACT team, other health care providers, and the community to maximize health care responses, quality, and cost-effective outcomes.
- Facilitates communication and coordination between members of the health care team, involving the patient in the decision-making process in order to minimize fragmentation in services.
- Educate the patient, the family or caregiver, and members of the health care delivery team about treatment options (including but not limited to CVT, HT, evidenced based protocols), community resources, psychosocial concerns, and case management, so that timely and informed decisions can be made.
- Assist the patient in the safe transitioning of care to the next most appropriate level.
- Advocate for both the Veteran and facility to facilitate positive outcomes for the patient, the health care team, and the facility.
Pay: Competitive salary, regular salary increases, potential for performance awards
Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
