Nurse Practitioner/ Physician Assistant

We are seeking a nurse practitioner (NP) or physician assistant (PA) for primary care rounding in a behavioral health facility in the Holyoke, MA area. This is a full-time role with no on-call requirements.
Schedule:- 4/5 Days per week
- 8am - 4pm
- No Call
- Some weekend and holiday coverage required
Responsibilities:
- Perform primary care in a specialty setting
- Manage chronic disease, diagnosis illness and medical conditions, and implement plan of care
- Order and interpret lab results
- Prescribe and monitor medications
- Consult with physicians and coordinate patient care with other healthcare practitioners
- Educate patients and their caregivers on coping skills
- Build relationships with your patients, patients family members, and facility staff to facilitate effective treatment delivery
- Approximately 15 consultations per day
Requirements:
- Primary care experience required
- New grads encouraged to apply
- Active and unrestricted MA NP or PA license or application in process
- Active and unrestricted DEA or application in process
- Excellent communication skills
