The C-YES program also provides Medicaid application assistance services to children who appear HCBS eligible and are either without Medicaid and/or have Medicaid but opt out of HH Care Management. C-YES also develops person-centered Plans of Care (POCs) and provides ongoing HCBS service coordination for those families who elect to remain with C-YES and opt out of more comprehensive case management provided by Health Home Care Management Staff.
