Job Summary and Qualifications

As a Valve Program Coordinator you will plan, coordinate, and implement the care of the valve patient in the outpatient setting. In addition, you will act as a liaison with the physicians, patient/families and ancillary departments.

Plan, coordinator, and implement care of the valve patients in the outpatient clinic

Review clinical progress of patients and coordinates communications to referring, valve and primary care physicians

Plan, organize and provide direct care to the valve patient

Facilitate the development of a patient specific teaching plan

Review patient's current health information, relay information and make referrals as necessary to the physician

Serve as a resource for the hospital staff regarding the care of the valve patient

Other duties as assigned

Requirements

Experience

Minimum of 2 years of experience as a Registered Nurse (RN) working with cardiac patients

Education

Associates degree in Nursing (ADN) from an accredited nursing program

Bachelor of Science in Nursing (BSN) – preferred

License and Certifications

Current State of Texas Registered Nurse (RN) license or Compact Registered Nursing License. If Compact License Texas license must be obtained within 90 days of hire

Basic Life Support (BLS) required as per St. David's Healthcare policy

Advanced Cardiovascular Life Support (ACLS) certification – preferred
