Summary

The Social Worker position is assigned to the St. Cloud VAHCS-Mental Health Integrated Care Community.

Provides comprehensive psychotherapy and care coordination services to Veterans for issues related to mental health and behavioral issues, substance use disorders, and other complex and diverse psychosocial problems.

Incumbent also provides mental health services via telehealth to the Alexandria, Brainerd, and Montevideo Community Based Outpatient Clinics as needed.

Duties

Clinical mental health functions include the following tasks:
Conducting behavioral health and mental health biopsychosocial assessments of Veterans and collateral contacts. Establish Diagnostic and Statistical Manual of Mental Disorders (DSM-5) diagnoses.

Creating comprehensive, recovery-focused behavioral health and mental health treatment plans with Veterans and in coordination with other disciplines.

Completing suicide/homicide risk assessments and safety plans, as required.

Providing clinically efficacious, evidence-based therapeutic services to include individual, couples, family and group psychotherapy/counseling services for Veterans.


Coordinating with internal and external and ancillary mental health professionals to ensure appropriate transitions of care and that appropriate and necessary treatment is provided/offered to Veterans.

Developing and providing psychoeducational groups.


Utilizing assessment and documentation processes for integrated models of care, including determining the appropriate level of care for Veterans not in specialty mental health treatment.

Educating and train other professionals and students in effective treatment modalities.

Establishing/maintaining expertise on VA services and benefits, and community resources

.Establishing/maintaining effective relationships with various Veteran agencies and coordinate care with appropriate community services.

Collaborating effectively with an interdisciplinary healthcare team.

Completing on-call duties, as assigned.

Maintaining data and statistical compilations to comply with CARF, Joint Commission, VA and health care system policy and procedures.


Providing comprehensive Mental Health Treatment Coordination (MHTC) services within a team of interdisciplinary providers: Coordinate and facilitate the Veteran's overall treatment, view the Veteran comprehensively and provide continuity among various services by linking VA and community resources into a continuum of care, incorporate both inpatient and outpatient care, unless treatment requires relocation to another facility, enhance the continuity of care in a recovery-focused, integrative, rational, proactive, and individualized manner, collaborative assessment of the Veteran's needs, strengths, and challenges, plan specific services to meet the Veteran's identified needs, link required services, monitor progress toward treatment goals, and advocate with the patient to ensure needs are met.

Incorporating a Recovery Model of Care to provide the framework for a collaborative relationship between the Mental Health Treatment Coordinator and the Veteran:

Recovery based goals enhance accessibility, accountability, and efficiency of services-guiding principles of the Mental Health Treatment Coordinator/Veteran relationship are self-direction and patient responsibility, the facilitation of strength, independence, and hope, and the empowerment of the Veteran in his/her personal health.

Providing health care education and/or referral to qualified health care educators to Veterans and their families.

Maintain accurate, timely, and complete clinical records in accordance with professional practice and VA administration guidelines.

Possess and demonstrate ability to communicate effectively, both orally and in writing with people from varied backgrounds.

Collaborating in performance improvement processes and comply with performance measures as required by the VA for specific population/program.

Conducting practice in accordance with the VA and discipline specific ethical requirements.

Practicing within the limits of current licensure or certification.

Independently completing of thorough biopsychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their families.

Liaising between Veterans and/or their families and VA and community resources in order to ensure thorough delivery of services.

Maintaining data and statistical compilations to comply with CARF, TJC, VA and health care system policy and procedures.


Maintaining knowledge of Veteran's benefits and services, community resources, and process for making appropriate referrals to community and other governmental programs or agencies.

Assessing for signs and symptoms of abuse, neglect and exploitation. The incumbent will report suspicion of abuse, neglect and exploitation when indicated according to VA policy.

Work Schedule:
Monday-Friday 8am-4:30pm evenings/weekends maybe required
