Job Description

Position Title: Part-Time Licensed Practical Nurse for ON CALL/Weekend Program Positions

FLSA Status: Non-Exempt

Department: Keystone Center

Supervisor Title: Keystone Center Neighborhood Manager

I. ROLE PURPOSE

Responsible for providing professional nursing care to residents.

II. MAIN RESPONSIBILITIES
  • Act as charge nurse for the assigned resident care neighborhood to ensure continuous clinical leadership. Provide guidance to paraprofessional staff, inform families, physicians, and departments of residents' needs.
  • Collect resident data and maintain accurate documentation of their condition. Record care provided, such as Wounds, MARS, TARS, Monthly Summaries, and more. Stay updated on the neighborhood EMR dashboard and address identified areas promptly.
  • Conduct clinical tests when necessary and administer medications and treatments. Monitor the effectiveness of medications and treatments.
  • Coordinate and conduct weekly rounds visits with physicians if required. Implement physician's orders and comply with regulations and nursing practices.
  • Participate in completing reports and rosters.
  • Guide staff in resident care and communicate residents' needs or changes in condition to families, physicians, and departments.
  • Maintain CPR certification and follow facility policies for CPR initiation.
  • Provide direct resident care, assist with daily activities, and encourage resident engagement in various programs.
  • Perform postmortem care when necessary.
  • Report to the next shift and receive updates on residents and any incidents. Accountable for narcotic counts at shift changes and reporting discrepancies.
  • Attend resident care conferences and review C.N.A. documentation.
  • Participate in orienting new nursing personnel.
  • Consult with supervisors or directors regarding nursing policies, residents, employees, or any concerns.
  • Maintain a clean, safe environment and support infection control programs.
  • Stay informed about current community initiatives, nursing procedures, and technology.
  • Practice fiscal responsibility in supply and equipment use, and time management.
  • Attend in-service programs and staff meetings.
III. ADDITIONAL RESPONSIBILITIES
  • Perform other assigned work duties and responsibilities.
IV. REQUIREMENTS
  • Graduation from an accredited school of nursing as a Licensed Practical Nurse.
  • Basic computer knowledge required.
V. LICENSES/ CERTIFICATIONS
  • Current LPN licensure by the NJ State Board of Nursing.
  • CPR certification preferred.
VI. PHYSICAL & ENVIRONMENTAL REQUIREMENTS

The physical demands include standing, walking, using hands, lifting, and more. Accommodations are available for individuals with disabilities.
