If you are contemplating our Social Worker, Care Manager role, your responsibilities would entail assessing the social, emotional, and environmental needs of Care Management members, designing social care plans, coordinating necessary community health services, and continuously monitoring member progress.

Requirements:
- Bachelor of Social Work (BSW)
- Three years of social work experience
- Current LBSW license in Alabama
- Valid Driver's License
- Proficiency in Microsoft Office Suite
Desired Qualifications:
- Master of Social Work (MSW)
- One year of experience in community resource coordination
- Active LMSW license in Alabama
- Certified Case Manager (CCM)
