_Summary of Essential Functions for the Position:_
Committed to Caring, the Hospice Scheduling Coordinator is responsible to ensure efficient scheduling of the clinical staff, patient care activities and communication with team members regarding scheduling activities and communication within the EMR system, complying with State and Federal regulatory and agency guidelines. This position ensures proper and accurate information is entered timely, and will assist with patient, staff, physician, referral source phone calls, providing backup to intake processing referrals, and support the Director of Nursing and Executive Director with administrative duties to include but not limited to IDG, billing, ordering of medical supplies, and customer care calls.

Job Responsibilities:
_
- Two (2) to three (3) years of previous related experience, hospice required.
Previous Consolo experience preferred
:
- Must be personable, persuasive, and detailoriented and have demonstrated success at working in a fast paced and sometime stressful environment.
- Ability to demonstrate good organizational skills, time management.
- Proficient with a computer and Microsoft Word, Excel, and Outlook software. Working knowledge of EMR database preferred.
- Effective communication skills, both oral and in writing, with excellent interpersonal skills.
- Current driver's license, automobile liability insurance and reliable transportation required.
- Acceptance and ability to demonstrate and support the core values and goals of Agency.
- Must demonstrate good customer relations skills and a commitment to providing quality service.
- Travel may be required.
Educational Requirements:
_**High School Diploma or GED Equivalent Required; 2 year Associate Degree preferred.

Working Conditions & Physical Requirements:
_**Working environment is quiet and can be stressful due to deadlines, multiple tasks and general compliance of law, rules and regulations.
The position requires visual acuity and dexterity, sitting, standing, some pushing, pulling, and lifting up to 25 pounds.
