Schedule :
Full time, benefits-eligible Monday through Friday 8am-5pm but will require flexibility based on client and program needs.

Pay :
$21 / hr

Program Mission


To recognize and meet the distinctive needs of youth ages 18-24 who are experiencing homelessness by providing stable housing, mental health care, case management, life skills training, and connection to community services and resources with the ultimate goal of assisting all clients in obtaining and sustaining safe and stable housing.

Position Summary


To provide consistent, individualized, and thorough case management services to clients served by the Tenant Based Rental Assistance and Rapid Rehousing programs with the end goal of clients obtaining/maintaining stable housing.

The position is housed out of the VOA Youth Resource Center and includes mobile work meetings with clients out in the community/in their homes.

The position will work to develop accessible housing resources and community partnerships to facilitate this purpose.

Essential Duties

Provide case management services to clients by assisting them in developing goals and a case management plan including but not limited to obtaining and maintain housing, medical care, mental health care, state and government benefits, and assistance with legal issues.

Attend staffing meetings; participate and present client information at said meetings to achieve greater client success through collaboration as part of a multi-disciplinary team.

Interact with all clients in a professional, trauma-informed manner while utilizing techniques such as motivational interviewing, Progressive Engagement, and appropriate boundary setting.

Collaborate with program manager in ensuring program fidelity, documentation accuracy, confidentiality, and intra-agency collaborations.

Develop client files including case management plans, referrals, and/or move on plans as appropriate utilizing a trauma-informed lens and with a client first focus.

In conjunction with the client, develop treatment, program/service, referral and/or discharge plans, as appropriate, in order to establish documented, continuity of care and as a bridge to additional services.

Utilize both internal tracking sheets and required case note web infrastructure in a timely and accurate manner.
Conduct at least one home visit per month for each client participating in a housing program.
Direct case management toward client being self-sufficient from housing program.
As directed, facilitate weekly groups for housing clients that offer skills and support.
Attend work on a punctual, regular, and predictable basis.
Utilize agency vehicles as needed to provide transportation to clients.
Perform other functions as necessary or assigned.

Qualifications

A degree in the field of social services and two years' experience working in the social services field or any combination of education, experience, and training which provides the following knowledge, skills and abilities:

Must be able to qualify and pass Utah Department of Human Services Case Management Certification within 60 days of being hired.

Fluency in the English and Spanish languages
Exhibits clear communication skills and cooperates with others.

Ability to uphold professional boundaries, confidentiality regulations, agency policies and procedures, and interact in a professional manner with a diverse workforce, clients, and the public.

Ability to utilize Trauma Informed Care and Housing First practices to act and make sound decisions within the scope of position responsibilities as well as reach out for supervision when necessary.

Willingness to accept supervision and direction.
Ability to move around the facility and interact with clients for extended periods.

Must be at least 21 years of age, possess a current driver's license, have a good driving record and be insurable on the agency liability policy.

Pass Utah DHS - Office of Licensing background screening for those working with vulnerable populations.
Proficiency in Microsoft Office.

Physical Demands
Ability to lift and carry 50 lbs.
Ability to move around the facility, interacting with clients for extended periods.
Work is generally performed in an office environment. May entail using a computer for extended periods of time.
