Works with director of outreach services to ensure consistent and high-quality service is maintained and to identify other outreach opportunities.

Bilingual (Spanish)Conducts tobacco prevention and cessation education and initiatives primarily in the Latino community.
Member of the Tobacco Initiatives team, assisting with the development and implementation of tobacco prevention and cessation work plan.1.

Actively demonstrates the organization's mission and core values and conducts oneself at all times in a manner consistent with these values.2.

Knows and adheres to all laws and regulations pertaining to patient health, safety and medical information.3. Works closely with community organizations to develop and plan for their health ministry. Makes available a wide range of prevention, health education and support services based on community needs.4. Provides wellness education programs on a regular and on-going basis. Provides screenings and education based on the test results.5. Serves as a liaison with Saint Joseph Health System (SJHS) and community agencies. Taps existing SJHS services and resources and coordinates their delivery to the organization. Actively searches for opportunities in the communityand accesses a variety of community resources.6. Schedules all program classes and informs director. Keeps attendance list of participants and provides outcome information to designated personnel.7. Measures outcomes and provides reports on a monthly basis to the director. Works with director to create and maintain appropriate databases and measurement tools.8. Functions as an integral part of SJHS Community Health & Well-Being services. Assists the director in developing innovative ways of reaching out to the underserved in their own neighborhoods.9. Utilizes effective group leadership skills. Maintains contact with appropriate site staff.10.

Participates and assists in the coordination of health fairs, educational presentations and other outreach activities per the request of the director of Community Health & Well-Being.

Promotes SJHS throughout the community.11. Establishes trusting relationships with patients/families while providing general support and encouragement.12.

Serve as a consistent point of contact for patients and families to improve appointment attendance and follow-up; provide basic motivational interviewing; set and track goals with patients/families.13.

Be knowledgeable about community resources appropriate to the needs of patients/families and help utilize these resources, including scheduling appointments, and assisting with completion of applications for programs for whichthey may be eligible.14.

Assist in the development and coordination of SJHS community health needs assessments and other public health surveys.15. Represents SJHS by participating in meetings, and if necessary, becoming a board member of local non-profit agencies and organizations.16. Performs other duties consistent with purpose of job as directed.
