POSITION SUMMARY
Assists in managing the activities of Transitional Care Management (TCM) nurse activities and Continuing Care Network (CCN) program. in alignment with the mission statement. policies and procedures of the Baptist Health System.

Manages, facilitates TCM program and supervises staff in the discharge planning and post-acute care services for patients with diagnosis of Congestive Heart Failure (CHF), Acute Myocardial Infarction (AMI) and Pneumonia (PN) and other targeted diagnosis groups.

Collaborates with the Case Management directors and senior leadership to support and implement activities and programs as defined. Coordinates care and services, of selected patient populations.

Designs plan of care to avoid patterns of frequent acute hospital or emergency department use, readmissions, and health status decline.

Completes established competencies for the position within designated introductory period. Other related duties as assigned.

POSITION QUALIFICATIONS

MINIMUM EDUCATION:
Bachelor's of Science Degree in Nursing from an accredited college or university

PREFERRED EDUCATION:
Master's Degree in Nursing, Health Administration/MBA, Nursing Administration from accredited college or university

MINIMUM EXPERIENCE:
5 years acute nursing and nursing supervisory experience, health care provider or health care company experience.

Working knowledge of computers and basic software applications used in job functions such as word processing, graphics, databases, spreadsheets, etc.


PREFERRED EXPERIENCE:
5 years' experience in Utilization Review, Discharge Planning and Medical Case Management in a hospital setting. Strong analytical and organizational skills. Working knowledge and ability to apply professional standards of practice in work environment. Knowledge of state regulatory, federal regulatory, and managed care requirements.

REQUIRED CERTIFICATIONS/LICENSURE:
Possession of current Texas State license for Registered Nurse

PREFERRED CERTIFICATIONS/LICENSURE:
Certified in Case Management

REQUIRED COURSES/COMPLETIONS (e.g.

, CPR):

Must have active healthcare provider Basic Life Support on hire (ARC or AHA); however, must have AHA Healthcare Provider BLS within 60 days of Hire
