Schedule:

This position offers 8-hour day shifts on Fridays, Saturdays, and Sundays, totaling 24 hours per week.

About Us:

We are dedicated to providing exceptional care management services in a home care and/or hospice environment. Our goal is to support patients and their families throughout their treatment journey, ensuring they achieve their health and end-of-life care objectives.

Responsibilities:
  • Conduct in-home psychosocial assessments
  • Collaborate with interdisciplinary teams
  • Develop care plans and goals
  • Coordinate patient care and community resources
  • Provide crisis intervention and counseling
  • Facilitate care conferences and family meetings
  • Ensure compliance with regulatory requirements
  • Maintain documentation per guidelines
  • Engage with community partners
  • Contribute to quality improvement initiatives
Requirements:
  • Master's Degree in Social Work
  • 2 years of relevant experience
  • Vermont LMSW Licensure or LICSW preferred
  • Excellent communication skills
  • Ability to work with diverse populations
  • Knowledge of medical and mental health diagnoses
  • Understanding of social work standards and care management
Working Environment:
  • Travel required
  • May work in various settings including clients' homes
  • Possible exposure to environmental factors
  • Varied physical requirements
