Minimum to Midpoint Pay Rate:$ $35.25 / hour
SummaryThe Respiratory Care COPD/Pulmonary Navigator is responsible for reviewing and tracking daily reports for COPD exacerbation admissions. Performing chart reviews for these patients to identify what support they will need prior to discharge. Utilizing internal telehealth platform to have a virtual visit to discuss and educate what resources are available upon discharge. During chart reviews identify any barriers for patients upon discharge and create a plan to contact patient within seven days of discharge to follow up and help navigate patient to any services needed. Under Medical Director supervision, place orders as needed for outpatient PFT, referral for outpatient COPD management program if appropriate, and referral order for Pulmonary Rehab. if appropriate, had. This individual acts as a liaison for the patient while addressing their questions and concerns about their post discharge support options. Collaborating with Care Transitions and Care Management, the COPD Navigator will identify the post discharge needs of the COPD patient and provide increased post discharge support.
The Navigator will focus on these areas of service: clinical services, community outreach, data research, navigate post-acute care.
The COPD Navigator operates under the direction of Director for Respiratory Care Services
RequirementsEducation:
Bachelors degree required.
Experience:
Minimum of 3 years experience as a Respiratory Therapist required.
Certification:
BLS (Basic Life Support) from the American Heart Association required.
License:
Licensed in Florida and credentialed by the National Board for Respiratory Care as an RRT.
Other:
Demonstrates commitment to learning. Basic computer skills; MS Windows, MS Word, MS Excel and MS Outlook.
Excellent communication skills with patients and wide variety of healthcare givers (RN, CNA, PT, MD). Active
participant and/or membership in community/professional/academic organizations is desirable.
