Position Overview

Salary: $35 per hour
Schedule: Overnights Full Time

The role of the Wellness Nurse is fundamental in ensuring the health and well-being of our residents. By providing continuous clinical monitoring and support, the Wellness Nurse collaborates with community nurses, caregivers, and medication personnel to uphold regulatory standards and deliver exceptional care. The ideal candidate will exhibit teamwork, flexibility, adaptability to change, and a nurturing demeanor.

  • Coordinate the resident admission process, encompassing screening, assessment, and care requirements
  • Ensure adherence to resident care plans
  • Administer medications and treatments according to physicians' orders
  • Facilitate coordination of care and services with external providers, including resident physicians
  • Thoroughly document care provided and resident observations, particularly significant changes
  • Demonstrate proficiency in state regulations, policies, and procedures

Requirements

Successful candidates for this position should possess the following qualifications:
  • Familiarity with state regulations, policies, and compliance procedures
  • Graduation from an accredited practical nursing school
  • Current nursing license and state registration (LPN or RN)
  • Preferred three years of experience in assessment and geriatric nursing
  • A welcoming attitude, willingness to learn, and genuine empathy for our residents
