We are in search of an experienced:

HOME CARE PHYSICAL THERAPIST

Job Description:

  • Delivers patient care services including direct treatment and follow-up.
  • Evaluates new patients with physician orders and devises suitable care plans based on the diagnosis.
  • Accurately documents patient care data in a timely manner, reflecting changes in the patient's condition.
  • Submits the patient visit schedule to the Scheduler on a daily/weekly basis.
  • Communicates effectively with internal and external contacts.
  • Explains company and insurance reimbursement policies to patients and families, and collaborates on treatment plans.
  • Collaborates with the Patient Care Coordinator for referrals to other disciplines.
  • Provides education sessions and supervises peers, support staff, and trainees.
  • Participates in research and assists in new employee orientation.
  • Job Requirements:
  • Must hold a bachelor's degree in physical therapy from an accredited school and be licensed in the state of practice.
  • Preferably, a minimum of one year of experience in a non-home healthcare setting.
  • Strong understanding of physical therapy principles, assessment, and treatment under physician orders.
  • Completion of at least twelve hours of continuing education annually.
  • Willingness to adhere to assigned travel and working conditions.
