Job Description
The Licensed Practical Nurse (LPN) plays a crucial role in delivering patient care by utilizing the nursing process. This involves data collection, assessment, prioritizing patient needs, and evaluating nursing care outcomes.
Key Responsibilities
In partnership with patients and healthcare team members, conducts initial and ongoing assessments to gather data relevant to individual patient needs.
Ensures timely and accurate data collection, adjusts collected data as patient conditions evolve, and communicates assessment findings to other healthcare team members as necessary.
Adheres to all documentation standards within the LPN role.
Analyzes assessment data, assesses patient conditions, and collaborates with providers to establish and manage patient care plans.
Provides patient, family, and staff education in collaboration with other healthcare organizations as directed by the provider or manager.
Administers medications safely in accordance with organizational policies, procedures, and the State of Ohio Nurse Practice Act.
Licenses/Certifications (must be current and active):
Current LPN licensure in the relevant state. IV certification may be required for specific specialties.
