  • Standard or Office w flexExempt from Overtime: Exempt: YesWhat You'll DoSummary:The Diabetes Nurse Educator performs professional nursing, consultation, and educational services for persons with diabetes for patients of all ages from infancy to geriatrics.
The Registered Nurse that is also a Certified Diabetes Educator applies in depth knowledge and skills in the biological and social sciences, communication, counseling, and education to provide individualized diabetes self-management education/ self-management training to individuals and families according to the Scope of Practice, Standards of Practice and Standards of Professional performance for Diabetes Educators (AADE, 2019).

Responsibilities:


1.Performs nursing assessments to gather data and identify the patient's needs for this episode of care per ANA standards and NCBON.2.Provide education related to diabetes and the disease process; medication, diet and nutritional monitoring; foot care, prevention and care of complications; blood glucose monitoring and techniques of self -monitoring and injection.3.Develop and implement educational programs for diabetic staff, patients, and families.

4.Advise patients as to long term complications.5.Provide clinical resources and consultation to staff and providers.6.Assists patients in monitoring blood glucose for the purpose of medication control and adjustment for both insulin and non-insulin diabetic patients.

7.Coordinates and communicates patient care needs for discharge planning in interdisciplinary care conferences and with members of the health care team.8.Develops and implements an individualized plan of care with the patient and family.

The plan of care will include educational goals, learning objectives and educational content and teaching methods.9.Collaborates with physicians and other health team members in coordinating the plan of care and facilitating patient outcomes.

Utilizes a team approach to provide services and collaborates and communicates with team members when needed.10.Provides ongoing assessment and recommendations to the treatment team and patient/family for adjustments in the plan of care.

11.Provides care based on established standards of practice, policies and protocols.12.Demonstrates leadership by providing information as a resource nurse, mentor and coach and delegator of nursing care activities.13.Demonstrates critical thinking skills for problem solving and anticipating patient care needs.

Documents assessments, delivery of care and patient's response to care, educational plan, educational services provided and evaluation results in each participant's medical and educational records.14.Provides information and facilitates patient/family teaching and identifies when a program participant's needs are outside the scope of the instructor's practice and expertise, plus arranges for additional services to meet needs.15.Demonstrates accountability for ongoing learning needs, completion of annual education requirements and competencies and participates in peer review process to evaluate performance of other professional instructional staff.16.Document assessments, interventions and plans including recommendations and goals, with educational tools.

17.Serves as Subject Matter Expert / Consultant for patients with recognized diabetes as well as subject matter expert when needed in the organization.

18.Participate in performance improvement and continuous quality improvement activities.

Experience:
Minimum of 1 year experience in caring for adults with diabetes and diabetes treatment management in a healthcare setting

Knowledge, Skills and Abilities:
Able to lead and effectively manage groups. Ability to develop a collaborative, therapeutic alliance with individuals and must possess basic computer skills (use of electronic medical record, internet, meter downloads and communication via email. Demonstrates standards of performance (ownership, teamwork, communication, compassion) that support patient satisfaction and principles of service excellence. Performs other duties as assigned. Individual will possess commensurate combination of education, experience and qualifications.
