Community based nursing allows Mercy Home Health to provide a personal touch to the patients we meet while seeing them through to success.
This nurse will be responsible for coordinating palliative care of patients in Home Care and advising/assisting those co-workers in identifying patients that need to be transitioned to Hospice.
This nurse may also case manage some patients from the transition period throughout their Hospice length of stay.Graduation from accredited nursing school.
Preferred Certifications:
Current certification as a Hospice and Palliative Nurse.
