Social Worker MSW - Department of Care Management at Providence St. Vincent Medical Center (Per diem, Day Shift with Weekend and holiday coverage)

$5,000 hiring/sign-on bonus for eligible external hires that meet required qualifications and conditions for payment.

The CSWA Social Worker - Department of Care Management, under direct and continuous supervision, provides comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems. The CSWA incorporates concepts and theories associated with life span development, neurobiology, and behavioral change. The CSWA will collaborate with the patient and support system(s) to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the Inter/Multi-Disciplinary Team (IDT). The CSWA shall perform duties in a manner that respects patient self-determination that reduces health disparities/barriers to care and adheres to the NASW code of ethics and Standards of Practice (SOP).

Required Qualifications:

  • Master's Degree in Social Worker
  • Oregon Clinical Social Work Associate License upon hire. Or
  • Oregon Clinical Social Worker License upon hire.

Preferred Qualifications:

  • 2 years of Clinical work experience within healthcare, hospice and/or bereavement.
