A licensed Registered Nurse (RN) position is available for our admissions team located in Manhattan. As a Hospice Nurse (RN) in this role, you will conduct assessments to admit patients into our hospice program.

About the Position:

  • Specialty: Hospice
  • Patient Population: Adults / Geriatrics
  • Schedule: Monday to Friday
  • Hours: Standard business hours

Responsibilities:

  • Perform thorough assessments of potential hospice patients to create customized care plans, conducted both in homes and facilities.
  • Admit eligible patients into the hospice program following set standards of practice by the National Hospice and Palliative Care Organization (NHPCO).
  • Execute additional tasks as required.

Requirements:

  • Minimum of 2 years' experience in med-surg, community health, oncology, or hospice.
  • Graduate from an accredited School of Nursing.
  • Active NYS Registered Nurse (RN) license.

Desired Qualifications:

  • BSN degree
  • Prior experience in hospice care
  • Previous involvement in intake or admissions processes
